Lake Atitlán is roughly 2.5-3.5 hours from Antigua by road, depending on the exact route and traffic through the highlands. That makes a day trip physically possible, but it means 5-7 hours round trip in a vehicle for a visit that's realistically only a few hours long once you actually arrive.
The honest answer: day trip vs staying over
If you're tight on time and just want to see the lake and volcanoes with your own eyes, a day trip works - most tours leave Antigua early morning and get you back by evening. But the lake rewards slowing down: boat-hopping between villages, watching the light change over the volcanoes at sunset, a Spanish class or two in San Pedro. If your schedule allows even one night at the lake, most people who've done both say it's worth it over a rushed day trip.
